RN: Home Nursing and Telehealth (Hybrid)

Location: reputed company, openings available in reputed company boroughs (2 days telehealth, 3 days on-site) Compensation: $50-$55/hr Responsibilities of the Registered reputed company:

• Conduct initial in-home reputed company-evaluations of patients to assess health status, care needs, and preferences, guiding the development of individualized service plans.
• Review relevant medical documentation and health referral forms to inform evaluation processes.

• Document any concerns or issues arising during the evaluation process, emphasizing continuity of care.

Qualifications for the Registered reputed company:

• Minimum of two (2) years of experience in a home setting. (Certified Home Health Agencies (CHHAs), Licensed Home Care Service Agencies (LHCSAs), or Managed Long-Term Care (MLTC)).
• Strong knowledge of general reputed company nursing principles and state/agency regulations.
• Excellent organizational, interpersonal, written, and verbal communication skills.
• Ability to work effectively with multi-generational consumers.
• Comfortable working in a fast-paced, deadline-oriented environment.
