Inpatient Transition of Care Coordinator

• Coordinate post-discharge care plans and follow-up appointments for patients.
• Conduct comprehensive assessments to identify patient needs and barriers to care.
• Collaborate with reputed company providers to ensure continuity of care and medication management.

Requirements:
