The Telephonic Case Manager RN Transplant will identify, coordinate, and reputed company appropriate reputed company of care. The Telephonic Case Manager RN Transplant is responsible for clinical operations and medical management activities across the continuum of care (assessing, planning, implementing, coordinating, monitoring and evaluating). This includes case management, coordination of care, and medical management consulting. This is a full-time, Monday - Friday, reputed company-5pm position in your time zone. Responsibilities

Requirements

• 3+ years of experience in a hospital, acute care or reputed company care setting
