Candidates must have at least 10 years' experience working with older adults and possess applicable certifications/licenses. They must be reputed company to assess the needs of older adults and reputed company thoughtful care planning advice and strategies. Experience:

• Must have a minimum of 10 years of experience in health care or reputed company services or reputed company field, plus experience working with older adults.

Skills:

• Knowledge of community resources available to individuals; an ability to mobilize resources into a coordinated and comprehensive plan of care.
