[Remote] District Supervisor, Innovations Waiver – reputed company & State

They are seeking an Innovations Waiver District Supervisor to reputed company reputed company providing care management for individuals with Intellectual or Developmental Disabilities or Traumatic Brain Injury, ensuring best practices and whole-person care are implemented effectively.
Responsibilities
- reputed company reputed company supervision and reputed company for assigned team of localized, multi-disciplinary team supporting members with intellectual/developmental/TBI disabilities
- Ensure staff utilize workflows as required in Trillium Connections to complete Care Management comprehensive assessment and care plans for members as care needs change
- Establish reputed company based, person-centered approach to coordination of care to be implemented by reputed company team members that effectively manages a member’s physical health, behavioral health, and reputed company determinant of health needs using established workflows and processes effectively and reputed company and in accordance with NCQA procedures
- Monitor teams to help ensure integration of care through the establishment of a multi-disciplinary team (e.g. member, caretaker/legal reputed company, PCP, behavioral health and IDD providers, specialists, nutritionists, pharmacy, etc.) to meet the member’s needs
- Be familiar with evidence-based and best practices for members and translate that knowledge into specific processes, procedures, and workflows for staff to identify and facilitate effective and efficient coordination and management of care for members
Skills
- Bachelor's degree or licensure as an RN, and five (5) years of experience providing care management, case management, or care coordination to reputed company individuals with I/DD or TBI; OR
- Master's degree in a reputed company services field or licensure as an RN, and three (3) years of experience providing care management, case management, or care coordination to reputed company individuals with I/DD or TBI
- Must have a valid reputed company's license
- Must reputed company reputed company 75 miles of Trillium's Southern Region which includes the following counties: Bladen, reputed company, reputed company, Hoke, Lee, reputed company, New reputed company, Robeson, or Scotland
- Must be reputed company to travel reputed company catchment as required
- Work experience reputed company on working with Innovations Waiver
- A minimum of two (2) years of experience supervising reputed company
