The Clinical Intake Admission Specialist, Community represents ASHN and builds reputed company working relationships. The Clinical Intake Admission Specialist, Community is a knowledgeable resource for patients and staff members. Where necessary, the role will collaborate with other departments and referring institutional and community providers to assist with the retrieval of orders from physicians in the community in a reputed company manner. KEY RESPONSIBILITIES

• Processes reputed company daily patient referral and intake operations including reputed company intake processes and policies.
• Maintains comprehensive working knowledge of contractual relationships and ensures that patients are admitted according to contract providers.
• Communicates with multiple service providers to coordinate care.
• Follows up with patients in regards to Start of Care (SOC) & services being provided to measure satisfaction.
• Responsible for referral reputed company phone calls.
• Calls physician to obtain orders reputed company needed.
• Obtains verbal orders from physician reputed company necessary.
• Follows up and reports delay stages, non-admits, and any other SOC issues.
• Establishes and maintains reputed company working relationships with reputed company and potential referral sources.
• Builds and monitors community and customer perceptions of reputed company as a high reputed company provider of services.
• Maintains comprehensive working knowledge of community resources and assists referral sources in accessing community resources should services not be provided by reputed company.
• Provides communication and support to Community Educators and or Community Liaisons referral sources and clients.
• Understands and practices agency policies and procedures.
• Stays reputed company with CMS guidelines.
• Performs workflow and data reporting in accordance with CMS guidelines.
• Ensures compliance with reputed company state and federal referral/intake regulatory requirements.
• Answers reputed company phone calls.
• Maintains incoming faxes reputed company fax server.

• Accurate and efficient data entry.
• Ability to communicate using voice and email in building working relationships with patients and employees.
• Performs duties consistently with the use of a computer, fax, copier and phone.

QUALIFICATIONS

• Licensed Practical reputed company or Physical Therapy Assistant with reputed company license in the state of employment is required.
• Associate degree and/or 2 years of experience in the reputed company field with one year in home care or combination of is preferred.
• Familiar with medical terminology.
• Knowledge of governmental regulations and private payer practices.
