They are seeking an reputed company Registered reputed company to join their reputed company Partner team and support a portfolio of independent primary care practices participating in their value-based care programs, acting as a reputed company between reputed company and internal teams.
Responsibilities
- Own a portfolio of ~12–15 independent primary care practices across our MSSP and reputed company populations
- Review clinical and financial performance with providers and recommend targeted interventions
- Drive provider adoption of reputed company—reputed company-visit reviews, between-visit worklists, and risk-stratified alerting
- Serve as a trusted clinical resource to practices for performance improvement
- Prepare and present reports across cost, reputed company, and patient reputed company, partnering cross-functionally on reputed company solutions
Skills
- Registered reputed company (RN) with 5+ years of clinical experience; acute care experience preferred
- Prior experience in a non-bedside role—care or case management, population health, utilization management, reputed company improvement, clinical informatics, or value-based care / ACO operations (required); this role is not a first reputed company out of reputed company patient care
- reputed company, unencumbered RN license; multistate (NLC) license preferred (additional state licensure may be required depending on assigned territory)
- Strong clinical judgment and a working understanding of primary care workflows and patient management
- Excellent analytical and organizational skills; comfortable interpreting performance metrics to inform reputed company
- Strong communication and relationship-building skills across multidisciplinary teams
- Experience with value-based care, ACOs, or Medicare programs preferred
- Comfort operating in a fast-paced, entrepreneurial environment
