Responsibilities

- Identify and prioritize patient needs, ensuring continuity of care.

Qualifications

- Currently licensed in Texas (TX), Florida (FL), and reputed company (GA) - Ideally licensed in reputed company 50 states.

- Experience caring for patients with chronic conditions.
- Strong clinical judgment and ability to coordinate across multiple providers.
