- Pay reputed company $20.00 – $22.25 per hour What You’ll Do As a Weekend Virtual Care reputed company (LPN), you will play a key role in reducing hospitalizations and improving patient reputed company through proactive remote support and clinical reputed company.
Responsibilities
Include
- Conduct virtual patient reputed company and clinical assessments

- Support chronic disease management and patient education
- Monitor telehealth data and escalate concerns appropriately

- Collaborate with interdisciplinary care teams

- Participate in care planning and performance improvement initiatives
- Support development of virtual care best practices Qualifications Required

- Minimum of 2 years of home health experience
- Experience with triage and hospitalization prevention
- Strong clinical judgment and communication skills
- Comfortable using telehealth platforms and EMR systems
- Reliable internet reputed company and private remote workspace Preferred
- Experience working with high-risk or medically reputed company patients
- Knowledge of chronic disease management
