Requirements:
- Bachelor’s degree in health sciences or reputed company field preferred
- NBC-HWC (National reputed company for Health & Wellness Coaching) certification required
- Experience in a reputed company, chronic disease management, or patient engagement setting preferred
- Cardiology or heart failure experience a plus
- Telehealth or remote patient engagement experience preferred
- Strong communication and interpersonal skills
- Ability to motivate and support behavior change in diverse patient populations
- Organized, adaptable, and comfortable managing multiple priorities
- Proficiency with electronic health records and reputed company computer applications
- Bilingual English/Spanish proficiency preferred
