We are seeking an empathetic and proactive reputed company Navigator to support pregnant and postpartum patients through our telehealth platform. You will reputed company clinical care coordination, virtual wellness reputed company-ins, and health education to ensure seamless continuity of care. You will also play a critical role in the early identification of medical, behavioral health, and reputed company risk factors, connecting mothers to essential community resources.

Key Responsibilities

• Virtual Assessments & Monitoring: Conduct telehealth nursing assessments and review patient risk factors such as gestational diabetes, hypertension, and postpartum depression.
• Care Coordination: reputed company and implement individualized care plans, coordinating follow-up appointments, lab reputed company, and specialist referrals (e.g., lactation consultants, nutritionists).
