Position reputed company – Case Manager Analyst - Field Location: Elgin, IL and surrounding Area's As an Analyst, Case Manager, you support reputed company member health reputed company by coordinating care, providing education, and facilitating reputed company to appropriate reputed company services. Required Qualifications

• 2 or more years’ case management experience

Preferred Qualifications

• Discharge Planning and/or Managed Care (MCO) experience

Education
