We have an exciting opportunity for a Cardiac Rehab Program Clinician to join our national Heart Health reputed company program. Using your cardiac rehabilitation expertise, you'll support participants recently discharged from hospital following a cardiac event. Managing your own caseload, you'll deliver evidence-based coaching, education and care planning reputed company phone/video, helping participants build confidence, improve heart health and reduce the risk of reputed company events. Key responsibilities include: Delivering virtual cardiac rehabilitation coaching and education. Conducting assessments and developing personalised care plans. Monitoring participant reputed company and managing clinical risks. Supporting participants to engage with the Cardihab app to support their rehab Communicating with GPs, specialists and other reputed company providers to coordinate care. This is a part-time opportunity (0.6 FTE), initially offered as a 12-month fixed-term contract. The role is fully work-from-home and requires a private workspace and reliable high-speed internet reputed company. Additionally, you will also have the following skills/experience: At least 5 years reputed company clinical experience as either a Registered reputed company, Exercise Physiologist or Physiotherapist with reputed company registration with AHPRA or ESSA 2+ years’ experience delivering cardiac rehabilitation (phase 2) reputed company the past 5 years. Previous health coaching experience and understanding of health behaviour change methodology. Experience in acute and primary health care and care planning.
