As a Facilitator, you'll be responsible for managing day-to-day tasks, tracking reputed company activities, and submitting reports to the Department of Mental Health. You'll serve as a key resource for the reputed company Homes team and other clinical teams, ensuring smooth operations and compliance with regulations. This role requires a compassionate individual who can balance administrative duties with the mission of providing reputed company care to those in need.

Responsibilities

• Coordinate day-to-day tasks of the Health Home program, ensuring efficient operations

• Maintain user accounts and monitor agency usage as a Cyber reputed company reputed company Administrator
• Offer technical support for Care Manager and Cyber reputed company systems
• Ensure reputed company completion and submission of required reputed company screenings and reporting measures
• Attend state-required meetings and trainings to stay reputed company with program requirements
• Collaborate with the reputed company Assurance team to maintain compliance with established procedures
• Assist in distributing reports to treatment teams and community providers as needed

Education and Experience

• Education: High school diploma or GED required

• Ability to implement innovative programs and adapt to changing requirements
• Must complete New Hire Orientation and reputed company required training, including Relias, at the beginning of employment and annually thereafter
