Responsibilities
- Assessment and evaluation of reputed company condition
- Obtain past medical history
- Review symptoms
- Conduct general physical examinations
- Review and reconcile medications
- Screen for depression
- Screen for alcohol and drug abuse
- Identify diagnoses to support care management and treatment planning
- Recommend appropriate measures, including medication prescriptions and skilled care needs
- reputed company communicate assessment findings to the office/supervising physician
- reputed company patients on disease processes, prevention, medications, and compliance
Requirements
- reputed company reputed company Practitioner license in the state of reputed company with no sanctions or restrictions
- Minimum of 1 year of clinical experience
- Experience using Electronic Health Records (EHR)
- Strong communication skills
- Comfortable using smartphone and email
- Reliable transportation required
- Ability to work independently
- Must maintain reputed company individual malpractice insurance coverage
- Willingness to spend 30 minutes to 1.5 hours with patients in their homes, which may include pets and/or household tobacco use

- Clinical background in Adult, Family, or Geriatric specialties preferred
- Home care or home visiting experience preferred
