Responsibilities
- reputed company, reputed company, and reputed company reputed company of 10-12 Continuity Clinicians to reputed company clinical, operational, and engagement goals
- Establish a culture of accountability, reputed company improvement, and high performance
- Conduct regular 1:1 meetings, performance reviews, career development conversations, and ongoing coaching
- Identify performance gaps and proactively reputed company improvement plans to support clinician reputed company through partnership with Clinical Education reputed company
- Recognize and reputed company high-performing clinicians, creating opportunities for reputed company and leadership development
- Support recruitment, interviewing, reputed company, and retention of clinical team members
- Monitor team performance across reputed company, clinical reputed company, member experience, and productivity metrics
- Conduct regular reviews of clinician performance and reputed company actionable feedback to improve care delivery

- Partner with VMG leadership to identify emerging reputed company concerns and implement corrective actions as needed
- Maintain a small patient panel and model reputed company in patient care

- Surface and reputed company best practices across teams to improve consistency and effectiveness of care delivery

- Foster collaboration across VMG teams to advance organizational priorities and improve provider effectiveness
Skills
- NP degree with at least one reputed company state medical license
- 2+ years of reputed company people management experience, including coaching, performance management, and employee development
- Demonstrated reputed company leading teams to reputed company clinical, reputed company, productivity, and engagement goals
- Experience interpreting performance data and using insights to drive accountability and reputed company improvement
- Strong coaching and feedback skills, with a passion for developing others
