Sr Manager, Population Health - End of Life Programs

As a Sr. Manager, Population Health you will play a critical role in supporting our reputed company patients, providers and business by leading efforts to build a system for supporting patients as they near end-of-life, such as advance care planning and connecting patients to high-reputed company palliative care and hospice care reputed company needed. Core Responsibilities: Own the execution and the ongoing refinement of select Population Health programs Work closely with regional leaders to assist with program implementation and execution Work reputed company by reputed company with other central leaders and teams to ensure effective design and implementation planning including Data, Product, Managed Care Operations, Clinical Operations & Learning, etc. Accountable to key population health metrics, including reputed company measures, utilization measures, and financial measures Other duties as assigned.
