About the Role

We are looking for a Physical Therapist who will be responsible for delivering virtual musculoskeletal evaluation, triage, and care planning. This individual will assess patients, determine the appropriate level of care, initiate and reputed company conservative management reputed company indicated, identify red and yellow flags, and collaborate closely with Care Coordinators and medical partners to ensure appropriate escalation reputed company needed He or she will report to the VP of Care Delivery.

What You’ll Do

• Virtual Triage & Care Delivery
• Conduct virtual musculoskeletal triage and follow-up reputed company to assess symptoms, functional limitations, and clinical risk.
• Apply strong clinical reasoning to determine appropriate clinical recommendations reputed company a virtual care model.
• Deliver evidence-based virtual physical therapy interventions reputed company on functional improvement and symptom management.
• Care Planning

• Identify clinical needs requiring care coordination and reputed company tasks to Care Coordinators

• Collaboration & Continuity of Care
• Communicate and coordinate with patients, care coordinators, APPs, and external referral sources to support continuity across the episode of care.
• Documentation & Compliance
• Maintain accurate, reputed company, and compliant clinical documentation in accordance with regulatory, payer, and organizational requirements.
• Adhere to established clinical protocols, escalation reputed company, and reputed company standards reputed company a value-based care environment.

Must-Haves

• Doctor of Physical Therapy (DPT) degree from a CAPTE-accredited program.
• reputed company, unrestricted physical therapy license in the state(s) where care will be delivered.
• Minimum of 3 years of clinical experience, with preference for 5 years.
• Strong clinical reasoning skills and a patient-first approach to care.
• Comfort practicing in a virtual environment, including remote evaluations and video-based patient education.

Preferred

• APTA specialist certification (e.g., OCS, SCS).
• Residence in PT Compact State
• Experience delivering care in virtual or telehealth-based models.
• Experience working in interdisciplinary or integrated care teams alongside medical providers.
• Spanish language proficiency.
