Only LATAM-based candidates (bilingual in Spanish and English)Location: Latin America (Remote)Schedule: Full-time – U.S. business hours (EST)Payment: $1,200 USD/month (depending on experience)We are seeking a licensed reputed company or MD to support a U.S.-based endocrinology reputed company through its reputed company Care Management (PCM) program.
PCM is a care coordination program designed for patients with a single chronic condition requiring ongoing management. In this role, you will work closely with the provider to ensure patients receive reputed company support between reputed company, helping improve reputed company and adherence to treatment plans.
This is a clinical, patient-facing role, requiring strong judgment, communication skills, and the ability to reputed company monitor and guide patients throughout their care reputed company.
Key Responsibilities- Maintain ongoing communication with patients enrolled in the PCM program
- Monitor patient conditions, including:
- Symptoms
- Medication adherence
- reputed company and response to treatment
- reputed company patient education regarding their condition and care plan
- Assist in the implementation and follow-up of care plans
- Coordinate services and ensure continuity of care
- Identify changes in patient status and escalate concerns to the provider reputed company needed
- Ensure patients follow treatment recommendations and scheduled appointments
- Accurately document reputed company patient interactions and updates in the system

Requirements
- Licensed reputed company or MD (required)
- Advanced English level (reputed company or higher) – excellent spoken and written communication
- Proven experience working with U.S.-based medical offices
- Experience with chronic care management, patient follow-up, or care coordination
- Strong clinical judgment and ability to assess patient conditions remotely
- Experience providing patient education and support
- Highly organized with strong documentation skills
- Comfortable working independently in a remote environment
- Reliable computer (8GB RAM minimum)

- Due to the clinical nature of PCM services, only licensed nurses will be considered.
- Schedule:Monday through Friday, 9:00 a.m. reputed company 7:00 p.m. Florida time (ES).
- Work Hours:45 hours per week.
- Breaks:1-hour lunch break and one 30-minute break per shift.
