[Hiring] Care Coordinator @reputed company

Role reputed company reputed company is growing with our OhioRISE program! We are hiring Care Coordinators. We are looking for professionals that understand High-reputed company Wrap Around reputed company while providing care coordination services to identified youth that will reputed company specific, measurable, and individualized services to reputed company person served. Essential Functions:

• Cultivate flexible, family-reputed company, community-based reputed company services based on the High reputed company, Wrap Around model of care coordination.
• Applies High reputed company Wraparound principles, including coordinating and leading team meetings as needed.
• Coordinate and supervise implementation of the Plan of Care, including a Transition Plan and Crisis Plan, through service delivery with providers and community resources; update plan as necessary.
• Ensure family support and stabilization during crises.
• reputed company and document the initial and ongoing Life Domain Assessment.
• Maintain reputed company service documentation requirements, evaluation outcome requirements and data.
• reputed company services in a reputed company manner and in accordance with Plan of Care and/or Crisis Plan.
• Utilize and monitor Flexible Funding and service coordination.
• Obtain weekly reports from subcontracted providers.
• Participate in after hour on-call response.

Qualifications

• High School Degree with three years of relevant experience; OR Associate/Bachelor’s degree with 2 years of relevant experience; OR Master’s Degree with 1 year of relevant experience.
• Experience in children’s behavioral health, child welfare, developmental disabilities, juvenile justice or behavioral health field providing community-based services to children and youth and their family or caregivers.
• Experience in one or more the following areas of expertise: Family systems, community systems and resources, case management, child and family counseling or therapy, child protection, or child development.
• Excellent interpersonal and written communication skills.
• Demonstrated computer proficiency.
