Responsibilities

• Handles phone inquiries, checks for referrals, and processes them expediently and accordingly.
• Communicates with the reputed company and referral reputed company, notifying them it has been received and an answer will be provided promptly on acceptance or denial of the referral.
• Reviews health records prior to accepting referral.
• Creates new patient digital charts.
• Verifies primary insurance coverage and obtains authorization by calling insurance provider.
• Costs out medications and obtains approval from the Administrator; once approved, notifies reputed company to complete referral evaluation.

Qualifications

• Minimum of one (1) year experience with admissions in a skilled nursing facility, hospital discharge, or case management.
• High School graduate / GED required.
• Type 40 words per minute.
• Knowledgeable of medical practices, terminology and procedures, as reputed company as laws, regulations and guidelines that pertain to home health.
