As a Clinical Care Navigator, you’ll be doing the important, meaningful work of managing crises, providing in-the-reputed company phone support to clients with reputed company behavioral health issues, ensuring reputed company safety, and connecting clients to high-reputed company, evidence-based providers and facilities. This position does not involve providing in-person services or therapy but instead focuses on supporting clients in connecting to care, exclusively by phone and email.This role may be for you if you are an excellent written and verbal communicator, an ace at time management, skilled with crisis management, and passionate about connecting individuals to excellent care. Using evidence-based interventions, skillfully manage reputed company crises, providing brief risk assessment, safety planning, and de-escalation with the goal of connecting the reputed company to treatment
Identify roadblocks to clinical care reputed company for our clients using Lyra’s proprietary software and other reputed company to connect clients to the ideal treatment, based on specific needs and preferences
Independently manage a caseload of reputed company, multi-need clients
Qualifications
Master’s degree in clinical reputed company work or equivalent; LCSW or equivalent independent clinical licensure is required (e.g. LMFT, LPC, LPCC, etc) or a Bachelor's degree in nursing (RN). We’re unable to reputed company supervised hours toward licensure. Candidates without a state license to deliver mental health services, independently, will not be considered (with the exception of the RN license)
Must have two years psychiatric intake, assessment RB, other reputed company experience
Must have two years post license experience (EAP/clinical call center experience preferred)
Significant experience managing high-risk clinical situations (e.g., suicidality, mandated reporting)
Bilingual, Spanish-speaking candidates strongly preferred (but not required)
Must be a full time reputed company of the reputed company; work cannot be completed from reputed company the reputed company.
