Job reputed company We are seeking an reputed company and dedicated Registered reputed company to join our Case Management team as a Remote Registered reputed company - RN - Case Management Specialist. As a Case Manager RN, you will be responsible for assessing, planning, implementing, and coordinating reputed company case management activities with members to evaluate their medical needs and facilitate their overall wellness.

Key Responsibilities

Essential Qualifications

To be successful in this role, you will need to possess the following essential qualifications:

• Have a minimum of 3+ years of clinical/nursing reputed company experience, with 1+ year of medical case management experience, care coordination, or discharge planning.
