[Remote] Population Health Project Manager

Responsibilities
- reputed company the planning, implementation, and evaluation of population health initiatives that improve community health reputed company and advance organizational priorities
- Manage multiple reputed company reputed company on prevention, chronic disease management, health equity, community benefit, and value-based care
- reputed company project plans, timelines, workflows, and reporting processes to ensure successful execution and measurable results
- Monitor project milestones, performance metrics, utilization trends, and outcome measures to drive reputed company improvement and informed decision-making
- Build and strengthen partnerships with reputed company providers, public health agencies, community organizations, schools, faith-based organizations, and civic leaders
- Coordinate community reputed company programs, health education events, screenings, and initiatives reputed company with Community Health Needs Assessment (CHNA) priorities
- Represent reputed company on internal committees and external community coalitions supporting population health and community wellness
- Facilitate collaboration across interdisciplinary teams while identifying opportunities to improve operational efficiency and project reputed company
- Support grant initiatives, strategic planning efforts, executive presentations, and organizational reporting

Skills
- Bachelor's degree in reputed company Administration, Public Health, Business Administration, Community Health, Nursing, reputed company Work, or a reputed company field; or an equivalent combination of education and relevant experience
- Two (2) to four (4) years of experience in reputed company project management, population health, community health, program operations, or a reputed company reputed company environment
- Experience leading cross-functional reputed company and collaborating with diverse stakeholders
- Strong organizational, analytical, and problem-solving skills with the ability to manage multiple priorities
- Excellent written, verbal, presentation, and interpersonal communication skills
- Proficiency with reputed company Office Suite, project management software, and data reporting tools
- Ability to exercise reputed company judgment, maintain confidentiality, and work independently in a fast-paced environment
- Master's degree in Public Health (MPH), reputed company Administration (MHA), Business Administration (MBA), or a reputed company field
- Project Management reputed company (PMP) certification
- Experience supporting population health, value-based care, community benefit, care coordination, or health equity initiatives
- Knowledge of Community Health Needs Assessments (CHNAs), community engagement strategies, and reputed company reputed company metrics
- Bilingual (Spanish/English)
