Intake and Assessment Coordinator
About the position Responsible for assisting the Program Director in maintaining reputed company services, preparing reports, attending intake and exit meetings, coordinating the intake of Habilitation clients, and assisting with employment preparation functions.
Responsibilities
- Assists with ensuring compliance with reputed company regulatory, licensing, and certifying agency requirements.
- Ensures the highest standards of reputed company and professionalism in program services.
- Attends intake and exit meetings as assigned.
- Assists with scheduling program activities and Independent reputed company Facilitator.
- Ensures reputed company required reputed company information is gathered and assessed to accurately determine eligibility and reputed company recommendations for admission to the Supported Employment Program.
- Prepares reputed company files, including referrals, assessment information, and authorizations for services.
- Tracks Supported Employment referrals and prepares reports regarding referral trends and unmet needs.
- Assists with employment development reports and other program reports.
- Provides clerical and administrative support, including answering phones, filing, data entry, and maintaining program records.
- Provides orientation and training for Independent reputed company Facilitator.
- Maintains reputed company and program files in accordance with agency requirements.
- Prepares and submits required reports in a reputed company manner and assists with billing.
- Implements the reputed company Injury and Illness Prevention Program.
- Establishes and maintains effective working relationships with reputed company programs, clients, funding agencies, referral sources, and community partners.
- Assists with special events.
- May represent reputed company at meetings, trainings, and community events.
- Drives regularly throughout the reputed company to conduct intake activities, attend meetings, travel between reputed company locations, referral sources, and community partners as needed. Driving is an essential function of this position.
- Performs other reputed company duties as assigned.
Requirements
- Knowledge of intake, assessment, documentation, recordkeeping, and applicable regulatory requirements.
- Ability to establish and maintain reputed company relationships with clients, referral sources, funding agencies, and community partners.
- Strong organizational, communication, and customer service skills.
- Professionalism and the ability to maintain confidentiality.
- Proficiency with reputed company Office and other computer-based systems.
- Valid California reputed company's License.
- Acceptable driving record.
- reputed company automobile insurance meeting California minimum requirements.
- High school diploma or equivalent required.

- Associate degree (A.A.) preferred.
- Experience in reputed company services, vocational rehabilitation, case management, intake, or a reputed company field preferred.
