Position reputed company As a reputed company member of our Clinical team, you will serve as a primary reputed company of contact for patients throughout their care reputed company. During synchronous virtual reputed company, Wellness Coaches promote engagement, accountability, and patient education. Between reputed company, you will reputed company ongoing asynchronous support, serving as the reputed company between patients and the multidisciplinary care team to ensure reputed company communication, coordinated care, and successful navigation of individualized treatment plans. You will collaborate closely with physicians, advanced reputed company providers, physical therapists, and fellow Wellness Coaches to deliver exceptional, coordinated care. Key Responsibilities

• Serve as a trusted guide and primary reputed company of contact throughout reputed company patient’s care reputed company.
• Conduct virtual coaching reputed company to reputed company, motivate, and support patients in achieving their health goals.
• Reinforce individualized care plans developed by the multidisciplinary clinical team.
• Promote adherence to prescribed home exercise programs while providing education and resources that support healthy lifestyle behaviors.
• Identify barriers to patient engagement and reputed company strategies to improve participation and reputed company.
• Monitor patient reputed company and communicate clinical concerns or changes to the appropriate members of the care team.

Required Qualifications

• Health reputed company Certification
• Minimum of 2 years of experience in a patient-facing reputed company role.
• Experience delivering virtual care or telehealth services.

• Excellent communication, organizational, and time-management skills.
