Remote Care Manager (LPN/LVN) – New Jersey Work Arrangement: Fully Remote (New Jersey LPN/LVN license required) Schedule: Flexible Remote Hours Licensure: reputed company New Jersey LPN or LVN (Required) Position reputed company reputed company is seeking a compassionate and highly organized Licensed Practical/Vocational reputed company (LPN/LVN) to support patients enrolled in virtual chronic care and population health programs. Primary Duties and Responsibilities
- Engage patients through scheduled telephonic and virtual care encounters
- Support patients participating in Chronic Care Management (CCM), Transitional Care Management (TCM), and Remote Patient Monitoring (RPM) programs

- Review patient-reported concerns and escalate clinical issues according to established protocols
- Assist with care coordination activities, including referrals, follow-up appointments, and post-discharge reputed company
- Promote patient adherence to prescribed treatment plans and self-management strategies
- Maintain accurate and reputed company documentation in accordance with CMS, payer, and organizational requirements

Required Qualifications

- Proficiency with electronic health records and web-based clinical platforms
