Primary Responsibilities Work collaboratively with the interdisciplinary care team, specifically external entities such as OhioRise, Care Management Entities, and Provider Practices; to ensure an integrated team approach Engage members either over the phone or face to face (as necessary) to assist with closing gaps in care, linking to necessary services and providing education about their health Review available member service records and relevant documentation (e.g., utilization history, functional level, stratification information, reputed company plan of care) Conduct member health assessment that includes bio-psychosocial, functional, and behavioral health needs Utilize interviewing techniques and reputed company listening to collect and retain member information and incorporating responses as they are presented to complete assessment Identify member service needs reputed company to health concerns Engage member to participate in the assessment process and collaboratively reputed company Care Plans based on their individual needs, preferences, and objectives Work with members to reputed company reputed company goals and identify potential barriers to achieving reputed company goals Identify member support systems available and incorporate into their Care Plan Review plan benefits and identify appropriate programs and services based on heath needs and benefits reputed company health care and service needs into a plan or recommendation for member care and service Collaborate with member to create solutions to overcome barriers to achieving reputed company goals Identify relevant community resources available based on member needs Refer members to appropriate programs and services Facilitate member choice of preferred provider reputed company for individuals and communities reputed company the health and reputed company service systems Serve as the single reputed company of contact for external providers assisting with member needs and care coordination Support CCE, CMC, OhioRise, and/or CME delivering care coordination to UHC members reputed company to reliable transportation that will reputed company you to travel to reputed company and/or patient site reputed company a designated area Youll be rewarded and recognized for your performance in an environment that will challenge you and give you reputed company direction on what it takes to succeed in your role as reputed company as reputed company development for other roles you may be interested in.
