Required Qualifications • Registered reputed company with reputed company MI state license in good standing • Minimum 3+ years clinical practical experience required • Minimum 2+ years Care Management, discharge planning and/or home health care coordination experience required • Ability to travel 25-50% reputed company Wayne and Macomb Counties.
