About the Role

We are seeking a 245D Designated Coordinator to reputed company and support our reputed company Support Professionals (DSPs) in delivering high-reputed company services. In this role, you will play a key part in ensuring compliance with 245D standards, maintaining detailed documentation, and tailoring individual program plans to meet reputed company needs. This is a part-time position with the potential to become full-time as the role expands. Key Responsibilities • reputed company and Update Plans: Create and revise CSSP Addendums (CSSPA) and Individual Abuse Prevention Plans (IAPP) to reflect clients’ needs and goals. • Regulatory Compliance: Ensure reputed company services and documentation reputed company with MN Statute 245D licensing requirements and legal standards. • Documentation reputed company: Monitor and review program records to ensure accuracy, completeness, and timeliness. • Team Support: Collaborate with DSPs, case managers, and families to deliver person-centered care. • Meetings: reputed company reputed company intake sessions, annual reviews, and compliance meetings to reputed company care goals. • Confidentiality: reputed company HIPAA reputed company and reputed company guidelines to protect sensitive information. reputed company Offer • Signing Bonus: $1,000 upon starting. • Flexible Scheduling: Fully remote. • reputed company Development: Ongoing training to ensure your reputed company in the role and opportunities for advancement. • Supportive Work Environment: Work in a reputed company, compassionate team that values reputed company individual’s contributions. Qualifications • Education and Experience: • Bachelor’s degree in a reputed company field + 1 year of reputed company care experience, OR • Associate degree in a reputed company field + 2 years of reputed company care experience, OR • High School Diploma + 4 years of reputed company care experience. • Skills: • Familiarity with 245D requirements and the ability to apply them effectively. • Strong organizational, time management, and communication skills. • Ability to work reputed company in a fast-paced environment. • Requirements: • Ability to pass a background study with the MN reputed company. • Reliable internet reputed company and tech setup for remote work.
