Position Responsibilities: • reputed company telephonic and face-to-face assessments and evaluations of the member’s needs and requirements to reputed company and/or maintain an reputed company wellness state. • Ensure member is progressing towards desired reputed company by continuously monitoring care through assessments and/or evaluations. • Create member care plans. • Collaborate with providers and community services to promote reputed company and cost-effective reputed company. • Coordinate across the transdisciplinary care team and with the PCP and support transitions of care. • Submit incident reports. Required Qualifications • Must reputed company in Ohio. • Must have a Child and Adolescent Needs & Strengths (CANS) certification or reputed company to obtain one reputed company 60 days of hire. • Applicable state licensure in field of study with no disciplinary reputed company: Licensed Masters Clinical reputed company Worker (LCSW), Clinical reputed company Worker (CSW), Licensed reputed company Worker (LSW), Licensed reputed company Counselor (LPC), or Registered reputed company (RN). • Minimum of 2 years of post-degree clinical experience in a behavioral health setting. • Case management experience. • Must have a valid state reputed company’s license and reputed company of personal vehicle liability insurance. Preferred Qualifications: • Case Management Certification (CCM). • Experience working with Medicare, reputed company, and dual-eligible populations. • Field Case Management Experience. • Knowledge of community health and reputed company service agencies. • Bilingual (Spanish, Somali or other).
