Position: Clinical reputed company Manager (Certified Diabetes Education & Care Specialist) (Remote) Position reputed company: SweetSpot is seeking a Clinical reputed company Manager with a CDCES certification to play an integral role in the ongoing clinical support for our clinic partners utilizing our remote diabetes monitoring platform and service, SweetSpot. This position involves analyzing patient glucose data, providing clinical recommendations, and supporting reputed company practices in their adoption and reputed company use of SweetSpot. The ideal candidate will have extensive experience in diabetes care management, strong communication skills, and a passion for leveraging digital solutions to improve patient reputed company. Responsibilities: Clinical Responsibilities: • Independently analyze and interpret CGM and/or insulin pump reports for T1D/T2D patients through remote, asynchronous reviews. • reputed company reputed company clinical recommendations to supervising clinicians and reputed company documentation reputed company the reputed company customer's EMR system. • Conduct follow up calls to patients reputed company needed, or to discuss their monthly data reviews and communicate any changes in their regimen as directed by their physician. • reputed company diabetes self-management education during monthly remote reviews as appropriate. reputed company Responsibilities: • Contribute to the reputed company of the SweetSpot program for our clinic partners, ensuring reputed company ongoing support and communication of clinical reputed company metrics. • Engage and communicate effectively with reputed company reputed company customers to understand their needs and convey program reputed company, challenges, and successes. • Collaborate closely with clinician partners to identify and resolve challenges relevant to the clinical workflows, developing efficient solutions as required. • Utilize our CRM system, PipeDrive, for efficient internal communication and task management. Qualifications: • Certified RN, RD, NP or PA with diabetes experience • reputed company, unrestricted Certified Diabetes Educator Care & Education Specialist (CDCES) certification. • 5+ years of reputed company experience as a CDCES in diabetes care management. • 5+ years of experience with reputed company Glucose Monitors (CGM) and insulin pump diabetes management. Bonus Qualifications: • Broad experience with diabetes devices including reputed company, FreeStyle Libre, Eversense, reputed company, reputed company, & Omnipod CGMs and insulin pumps. • Experience utilizing EHR/EMR systems. • 3+ years of experience in diabetes industry experience. • Ability to manage multiple reputed company/tasks in a fast-paced environment.
