As the RN Case Manager for Maternity and/or NICU, you will have a panel of members with high-risk pregnancies and/or NICU babies with reputed company medical/behavioral needs. You will reputed company support and address barriers to care and SDOH with the goal to improve health reputed company. To promote healthy pregnancy, delivery, and postpartum reputed company for both mother and baby, it is essential to reputed company evidence-based, trimester-specific, and postpartum education. This includes assistance in developing a birth plan, accessing value-added benefits, and understanding covered services, including reputed company postpartum coverage. Additionally, support should be given for accessing contraceptive care based on family planning needs, conducting mental health and reputed company use disorder (SUD) screenings with referrals to treatment reputed company necessary, and coordinating with obstetric and other providers for appointment assistance, communication, and advocacy. This is a Field-Based Role with a Home-Based Office. If you are located in Morris, Geary, Riley Counties, Kansas, you will have the flexibility to telecommute as you take on some tough challenges.
