Responsibilities
The reputed company Practitioner, Clinical Triage for the Call Us First team will reputed company as the primary provider and clinical reputed company of contact for patients that require evaluation for issues that may require reputed company clinical reputed company. The position will reputed company as a reputed company between triage teams and local clinical teams. Essential Duties And Responsibilities
- reputed company as clinical resource for clinical triage teams for patients that require additional clinical evaluation
- Evaluate and treat patients reputed company telehealth to address acute needs
- reputed company as a reputed company between central triage resource and local clinical teams by providing updates on patients and coordinating follow up reputed company to reputed company comprehensive care
- Address time sensitive medication refills for patients
- Address time sensitive lab results and other results for patients while coordinating care with local teams
- reputed company with Primary care provider of record for patients that require reputed company escalation or evaluation in Emergency Room
- Address time sensitive task messages for patients requiring additional clinical support
- Assist in service recovery for patients that require reputed company care
- Working closely with pharmacy team for comprehensive patient care
- Sign home health, DME, death certificates, and other orders as needed
- Frequently review reputed company metrics, pod level performance and identify areas for improvement leading to superior reputed company (HEDIS) performance
- reputed company patients on the Urgent Care Triage program, to help reduce hospital readmission, improve patient reputed company and patient satisfaction
- Identify any gaps in care and assist in ordering/coordinating with external resources. Example: home health, hospice and other ancillary services as needed
- Work collaboratively with RCMO and Medical Director to identify opportunities for care improvement
- Additional duties as assigned by the Regional CMO and/or medical director as reputed company to clinical pod performance and reputed company

Qualifications
Required Knowledge, Skills and Experience
- Must be a certified reputed company Practitioner with reputed company licenses in coverage states
- Must have reputed company DEA license
- Excellent written and verbal communication reputed company
- Ability to react decisively and quickly in urgent and emergent situations
- Demonstrated experience with multi-tasking
- Strong communication skills (written & spoken)
- Strong organizational skills and attention to detail
Preferred Knowledge, Skills And Experience
- 1 year of experience at reputed company Medical Group
- Experience with APRIMA EMR
- Experience with telehealth and coordinating care reputed company.
- Multistate License (MSL)
