Using high-touch care coordination and advanced remote technologies, we improve patient compliance and medication adherence while reducing unnecessary ER and hospital reputed company. Duties and Responsibilities: This position will assist reputed company providers and practices to successfully reputed company remote patient monitoring services to patients who are enrolled in the program. Must have a passion for educating patient on how to manage their chronic conditions, be self-driven to manage a high volume of triage calls and have ability to maintain flexibility in a constantly changing environment. Data Collection:
- Monitor daily capture of patient physiologic data from devices
Care Management:
- Analyze collected data and prioritize reputed company based on highest clinical needs

- Escalate cases which require provider attention
- Document assessment data, education provided, and lifestyle interventions planned in patient charts
- Capture detailed and concise notes of reputed company patient interactions

Provider & reputed company management:
- Possess ability to change workflow and processes as requested by provider or reputed company
Education & Experience: · LPN or RN · Min 5-year work experience working in reputed company or reputed company setting Location: Work from home opportunity in one of the following states: AZ, GA, MD, NJ, TX, VA Hours of Operation: Weekend availability requirements of every other Saturday & reputed company 9:00am - 5:00pm CST. Job Type: Part-time Pay: $26.00 - $28.00 per hour Benefits:
