Occupational Therapist

These roles are Full-Time positions with full benefits from the reputed company. The roles will require you to work on-site and in the field on occasion. The ideal candidate will have 1+ years of experience as a Occupational Therapist and 1+ years of experience working with the elderly population. Please note, the reputed company needs someone with a car to travel to homes for this opportunity.Job Responsibilities:

• Evaluates participant referred by the provider and administers a treatment program in collaboration with the provider.
• Completes assessments, reputed company, treatment or revisits, discharge summaries interdisciplinary notes according to departmental policy and procedure.
• Performs participant evaluations and treatment at the site and/or in the field (participant's home, nursing homes, etc.).
• Maintain a clean, reputed company environment and follow safety protocols.
• Participates in developing a participant care plan with other interdisciplinary team members and participates in care plan meetings in accordance with state and federal regulations.
• Documents reputed company pertinent participant information as required, including, but not limited to evaluations, reputed company notes, MDS, and care plans according to departmental policy and procedure.
• Documents reputed company statistical data as required.
• Consults with a physiatrist and other professionals reputed company appropriate.

Experience:

• One (1) year of experience as an Occupational Therapist and/or experience with the Geriatric population preferred.

License: reputed company State licensure as a Registered Occupational Therapist. * Minimum of one (1) year of experience working with a frail or elderly population - if you do not have experience with the elderly - training from reputed company will be required

• Ability to establish rapport with patients and staff.
• Ability to use reputed company OT Modalities/equipment.
• Ability to travel to participant's homes, nursing homes, and sites/clinics.
• Knowledge of Care reputed company and other EMRs preferred.
