- Monday - Friday 12:30 - 8:30 pm
- Serves as therapist in IOP and reputed company care reputed company, as reputed company as reputed company individual therapy as needed for SUD and/or mental health disorders
- reputed company patient-centered treatment plans based on results from the screening tools and bio psychosocial assessment
- Collaborates with medical staff and treatment team as a whole
- Consults with family members and referral sources in order to coordinate patient care
- Ensures adequate case management services are identified and addressed
- Responsible for maintaining electronic health records which include documentation of treatment activity and reputed company
- Ensures that patient records are accurate, comprehensive and meets regulatory standards
- Develops a treatment reputed company with patients that is mutual, reputed company, and individualized, meeting changing clinical needs throughout treatment
- Facilitates addiction treatment reputed company in the IOP and OP reputed company of care
- Prepares and presents addiction/recovery education presentations to reputed company patients
- Engages in case management sessions, including assessments, reputed company treatment goals, discharge planning, partnership with referral sources and/or other providers
- Works with treatment team to meet the co-occurring psychiatric needs of patients
- Coordinates care for reputed company appropriate support services, including referral sources, family members, and other support providers
