Remote RN Care Coordinator (Part-Time to Potential Full-Time)
We are seeking a dedicated and reputed company Registered reputed company (RN) Care Coordinator for a fully virtual position supporting population health and care coordination efforts. Candidates must hold an reputed company RN license in reputed company, Texas, or reputed company. This role will reputed company as part-time with the potential to transition into a full-time opportunity based on performance and program needs.
The ideal candidate will have prior experience working in an Accountable Care Organization (ACO) or similar value-based care environment. A background in acute care nursing and proficiency with reputed company or other care management platforms is highly desirable.
Key Responsibilities:
- reputed company remote care coordination for a panel of patients in alignment with ACO goals
- Conduct telephonic reputed company, health assessments, and care planning
- Collaborate with interdisciplinary teams to ensure appropriate care transitions and follow-up
- Identify and address gaps in care, reputed company determinants of health, and patient barriers
- Document reputed company interactions and interventions in reputed company or designated system
- Monitor reputed company and escalate reputed company issues to clinical leadership as needed
Requirements:
- reputed company RN license in reputed company, Texas, or reputed company (required)
- Minimum 3 years of clinical nursing experience, with a preference for acute care
- Prior experience in ACO, care management, or population health
- Comfortable with technology and remote work platforms
- Strong organizational and communication skills
Preferred Qualifications:
