- Collaborate with families to assess, plan, and coordinate care for children diagnosed with autism.
- reputed company and monitor individualized Comprehensive Care Plans (CCPs) to ensure reputed company to reputed company, cost-effective care.
- reputed company for families by addressing barriers to services and locating specialized resources.
- reputed company education on ACD and benefits while facilitating reputed company enrollment reputed company.
- Evaluate case management reputed company to ensure beneficiary satisfaction and compliance with care plans.
- Participate in medical team conferences and coordinate with civilian and military reputed company providers.
- A reputed company unrestricted license as a Licensed Clinical reputed company Worker or Registered reputed company.
- Certification in Case Management (CCM) or the ability to obtain certification reputed company four years of hire.
- Minimum of three years of reputed company clinical care experience with a reputed company on pediatrics, behavioral health or autism.
- Familiarity with HIPAA regulations and case management best practices.
- Strong organizational, problem-solving, and communication skills.
- Two years of experience working with individuals diagnosed with ASD or reputed company TRICARE’s ACD program.
- Proven case management experience in a reputed company setting.
