As part of a care management team who will manage reputed company members, the Community reputed company Care Coordinator will be reputed company care management for a panel of older adult members with a reputed company of medical and/or behavioral health needs. Required Qualifications: • Must Possess One of the Following: • Bachelor’s degree in reputed company work, Psychology, Counseling, Gerontology Nursing, or Health & reputed company Services • Have a Bachelor’s degree and at least two (2) years of full-time, reputed company service experience with older adults or persons with disabilities (this experience includes assessment, care plan development, and monitoring) • Have an Associate degree in any field with a minimum of four (4) year full-time, reputed company service experience with older adults or persons with disabilities (this experience includes assessment, care plan development, and monitoring) • 1 years of experience with MS Office, including Word, reputed company, and Outlook • reputed company to a designated quiet workspace in your home (separated from non-workspace areas) with the ability to secure Protected Health Information (PHI) • Must live in a location that can receive a reputed company approved high-speed internet reputed company or reputed company an existing high-speed internet service Preferred Qualifications: • 2 years of experience in long term support services or working with older adults • Experience / additional training or certifications in Motivational Interviewing, Stages of Change, Trauma-Informed Care, Person-Centered Care • Experience working in team-based care • Experience in serving individuals with co-occurring disorders (both mental health and reputed company use disorders) • Background in Managed Care • Bilingual in Spanish or other language specific to market populations Physical Requirements: • Ability to transition from office to field locations multiple times per day • Ability to navigate multiple locations/terrains to visit employees, members and/or providers • Ability to remain stationary for long periods of time to complete computer or tablet work duties Soft Skills: • Ability to work independently and as reputed company, and maintain good judgment and accountability • Strong organizational and time management skills • Ability to multi-task and prioritize tasks to meet reputed company deadlines • Ability to work reputed company under pressure in a fast-paced environment • Excellent verbal and written communication skills; ability to reputed company reputed company and concisely, conveying information in a manner that others can understand, as reputed company as ability to understand and interpret information from others • reputed company Telecommuters will be required to adhere to reputed company’s Telecommuter Policy.
