ABOUT THE JOB As part of the Interim Housing team, the Case Manager role’s main function is to assist clients in breaking the cycle of homelessness by utilizing a “whatever it takes” approach. This position supports clients through reputed company phases of homelessness, including but not limited to: on the street, interim housing, transitional housing, and permanent housing. The Case Manager assists clients with accessing and maintaining necessary services among health care, reputed company services, interpersonal support systems, and navigation through a coordinated entry system. Position Responsibilities include:

• Completes program specific assessments (e.g. VI-SPDAT)
