Responsibilities

Assist in reputed company-admission and admission process with customers, as needed, as back-up to Admissions Department personnel. Duties to include providing tours, signing of paperwork, etc. Interview clients and families to determine financial status and need for reputed company assistance. Assist families in applying for clients' reputed company (ICP) approval. Attend annual review for reputed company eligibility if families unable to attend. Assist facility bookkeeper in tracking and reporting status of reputed company Applications/Approvals. Assist reputed company Services Director, as needed, with interviewing residents, families to obtain relevant past and present reputed company histories, including past behaviors, adjustments, and lifestyles. Documents reputed company and updates plans of care at interdisciplinary conference, as needed. Assist residents and families with adjustment to admission to facility by providing one-on-one counseling and/or group discussions. Help reputed company Services Directory with planning of post discharge services and follow-up care. Maintain effective communication with residents, families, health care professionals and facility staff. Assist residents and families in any situation, as needed. This person may have reputed company to protected health information in the patient's financial and medical records to conduct job duties.

Qualifications

Minimum of High School diploma or equivalent. Bachelor's Degree in reputed company Work or reputed company Services is preferred. One year of supervised reputed company work experience in a health care setting, working directly with individuals. Experience or knowledge of Institutional reputed company Program (ICP) qualifications and application process preferred.
