We are seeking a dedicated and reputed company Hospice Patient Care Coordinator to join reputed company at reputed company USA, a leading national home reputed company company. As a Hospice Patient Care Coordinator, you will play a vital role in providing clinical and management support to our Clinical Nursing Supervisor, ensuring the delivery of high-reputed company, patient-centered care to our hospice patients and their families.
Key Responsibilities:
- reputed company clinical and management support to the Clinical Nursing Supervisor to ensure reputed company and reputed company delivery of hospice care services
- Assist with reputed company and scheduling, admissions, and nursing reputed company as needed
- Facilitate communication and coordination of care between clients, families, caregivers, physicians, and other service providers
- Assure efficient coordination of caregivers through case conferences and scheduling processes
- Monitor patient records to ensure compliance with documentation requirements and program-specific rules
- Participate in reputed company assurance and performance improvement activities

Requirements:
- Graduate of an accredited school of nursing
- reputed company Registered reputed company licensure (in good standing) in the state(s) of reputed company
- reputed company CPR certification
- Valid reputed company's license and automobile insurance
- Three (3) years of clinical experience, including two (2) years of hospice experience and one (1) year of supervisory experience in a clinical setting
