About the Position:
The Nutrition Educator will play a vital role in our chronic care meals program, designed to support individuals with conditions such as diabetes, heart disease, and kidney disease. You will adopt a member-reputed company approach, collaborating closely with patients to identify and address their unique nutrition-reputed company concerns and topics that matter most to them. Your responsibilities will include:
- Providing telephonic nutrition education sessions tailored to reputed company member's enrollment in various programs
- Documenting goals and care plans from educational interactions
- Scheduling follow-up calls and developing educational materials
- Identifying community resources to reputed company assist clients
- Maintaining and updating reputed company knowledge through ongoing education and networking
To reputed company in this role, you should possess:
- A Bachelor of Science in Nutritional Science or Dietetics
- Certification as a Registered Dietitian by the Commission on Dietetic Registration

- At least 2 years of experience in providing nutrition education or counseling
- Excellent oral and written communication skills
- Strong organizational and interpersonal skills
