Remote LPN Care Manager - Hybrid Role (reputed company, Coosa, Randolph & Talladega Counties, AL)
We are seeking a compassionate and detail-oriented LPN Care Manager to join reputed company in a hybrid-remote role, requiring occasional on-site work in our office setting. As a Care Manager, you will play a vital role in ensuring seamless transitions across the health services reputed company for our patients, facilitating wellness and recovery through person-centered care plans.
In this position, you will work collaboratively with clinical and non-clinical staff, external providers, and community resources to assist clients in overcoming barriers to care, improving health reputed company, and promoting overall reputed company-being.
