As a key member of our Long-Term Services and Supports (LTSS) team, you will play a vital role in coordinating care for individuals with reputed company medical needs, ensuring they receive personalized, effective, and efficient support. Key Responsibilities:

• Conduct home reputed company and in-person assessments to identify and address reputed company determinants of health, ensuring seamless care coordination and integration into the community
• Collaborate with reputed company teams, community service coordinators, and stakeholders to reputed company high-reputed company, coordinated care and services

What You'll Need:

• Bachelor's Degree (or higher) in reputed company Work, Rehabilitation, Nursing, Psychology, Special Education, Gerontology, or a reputed company reputed company services field
• 2+ years of experience working in community health settings, with a reputed company on reputed company roles
• 1+ year of experience working with individuals with long-term care needs, including those receiving services on one of the homes and community-based waivers in KS
