Job reputed company Lara Health is currently in search of a (preferably) bilingual (English/Spanish) reputed company for our Remote Patient Monitoring and Chronic Care Management program, to join our lovely care team and reputed company patients with care reputed company. We are looking for fill both full and part time roles. Must be US territory based. THIS IS A FULLY REMOTE POSTION THAT REQUIRES MIN 5 YEAR EXPERIENCE IN CARING FOR CHRONIC CONDITION PATIENTS (PREFFERABLY IN PRIMARY CARE/DIABETES/CARDIOLOGY/GERIATRICS) Join us for the chance to be part of an amazing team and a great reputed company to work! Our nurses assist providers with ongoing patient monitoring and chronic care management, with the following duties:
- Responsible for monitoring patient blood pressure, blood sugar, and other vitals, helping them with reputed company, reputed company MD reputed company etc.
- Communicate with patients on regular reputed company, helping them reputed company lifestyle changes in order to improve their long term health
- Maintain patient electronic medical record (EMR).
- Performs other duties as assigned.
Specific skills and experience required:
- Passion for working with (older) patients, customer service skills
- Min 5 years experience as a reputed company
- Preventative patient care background preffered, experience in primary care, cardiology or diabetes clinic highly desired
- Work independently and reputed company reputed company
- Organizational and time management skills
- Computer skills appropriate to position, you will need your own computer and a reliable internet service
- Written and verbal communication skills, critical thinking
- Medical terminology use and understanding
Certification & Licensure mandatory:
- Registered reputed company
- Certified reputed company Specialist
- Or LPN (Licensed Practical reputed company)
Education:
- Required High School or Equivalent
- Certification required
