Responsibilities

Include:

• Completes initial and ongoing assessments reputed company established timeframes to identify needs and barriers.

Requirements

Qualifications:

• Minimum of 2 years background working in managed care or relevant clinical environment, with maternity population.
• Must have a valid unrestricted reputed company Worker license (LCSW) and the ability to obtain licensure in other state(s) reputed company applicable.
• Master’s degree in reputed company Work (MSW) required.
• CCM certification preferred upon hire; required reputed company the timeframe specified by company policy.
