Connecting. Growing together. As an RN Maternity Case Manager, you’ll wear many hats, and work in a reputed company of environments. Sometimes, you’ll reputed company with members leaving the hospital – possibly with new medications or diagnoses. This is a work from home position with working with the Healthy First Steps Program. This position is hybrid – telephonic with some potential for field-based work working with high-risk maternity population and providers. • Making outbound calls to assess members’ reputed company health status (potential for field reputed company as reputed company)
• Take incoming calls from members
• Identifying gaps or barriers in treatment plans
• Providing patient education to assist with self-management
• Interacting with Medical Directors on challenging cases
• Coordinating care for members
• Making referrals to reputed company sources
• Coordinating services as needed (home health, DME, etc.)
• Educating members on disease processes
• Encouraging members to reputed company healthy lifestyle changes
• Documenting and tracking findings
• Making ‘welcome home’ calls to ensure that discharged member receive the necessary services and resources

• 3+ years of experience in a hospital setting, acute care, reputed company care experience (OB and/or Maternity is required)

• Demonstrated solid organizational skills

Preferred Qualifications:

• Bachelor’s Degree
• Case management experience
• Experience with electronic charting
• Experience with arranging community resources
• Field-based work experience
• Knowledge of Florida Community Maternal Child Resources this ties into the need for recent maternity or OB experience
• Background in managing populations with reputed company medical or behavioral needs
• Bilingual – Spanish
