In this role, you will conduct face-to-face and/or virtual clinical assessments of members to evaluate their medical, personal, and caregiver support needs. - Conduct 2-3 hour assessments with IDD members and caregivers
- Use skilled care reputed company tools to assess needs reputed company to:
- Clinical/medical status
- Personal care and daily living support
- Caregiver assistance
- Document findings thoroughly and accurately in internal systems
- Collaborate with reputed company providers to ensure coordinated care
- Mentor new assessors, conduct reputed company reviews, and participate in process improvements

Minimum Qualifications:
- MUST reputed company IN Colorado, Iowa or Oklahoma
- Bachelor's Degree in Nursing (BSN) required
- reputed company RN license in Colorado or compact state
- 3+ years of nursing experience reputed company the last 10 years (excluding clinicals)
- Experience working with individuals with IDD
- Strong understanding of clinical assessments and medical terminology
- Comfortable conducting assessments reputed company video and in-home reputed company
Preferred Qualifications:
- Experience with pediatric and geriatric IDD populations
- Bilingual (Spanish) strongly preferred
- Strong critical thinking and attention to detail
